Office Policies

  • We are a private pay practice which means that we do not accept insurance. We do however provide receipts in order for our patients to submit for re-imbursement from their insurance companies
  • Please call us 24 hours in advance if you are not able to make an appointment. This allows us to fill in your appointment time with another patient who is in need of help.

In order to help you start your appointment on time, please arrive 15 min prior to your initial appointment to fill out your new patient forms, or download the forms here. You can print, fill out, sign and bring these forms to your appointment. Thank you!
